Do you need help identifying a dental implant?

OsseoSource Facebook community was established to provide a place where dentists, dental technicians, and other dental professionals can seek help in identifying unknown implants. Recently we successfully helped Phil identify such an implant. Phil sent this request: 

"I am trying to identify this particular implant in #9 position so that I can order a replacement screw. I will attach a radiograph of the implant and a picture of the abutment interface. Can you please help me identify it? I appreciate whatever help you can give me."
